Family Support Services in Mental Health

Crisis affects more than the individual facing it—it impacts the entire family unit. At Hope Arise, we understand that families deserve comprehensive guidance, effective communication, and unwavering support as they navigate some of the most challenging phases of the healing process. 

When a client’s needs necessitate a higher level of care, such as inpatient hospitalization, Hope Arise is here to assist both clients and families in understanding their options, identifying appropriate resources, and facilitating the transition into treatment. With the necessary authorizations in place, our care coordination services can help ensure support during the treatment process and aid in preparing for the client’s return to outpatient care.

Support That Continues Through the Transition

At the onset of therapy, clients have the option to enter into a Hospitalization Support & Care Coordination Agreement. This voluntary service offers an added layer of support in the event that a client’s needs evolve or become more acute, necessitating a referral to a higher level of care. Establishing this agreement at the beginning of therapy allows clients and families to gain a clear understanding of the additional support available should hospitalization or another higher level of care be required.

For clients opting for this enhanced support, Hope Arise may assist with:

- Exploring appropriate levels of care and available treatment resources.

- Helping clients and families navigate the hospitalization process.

- Guiding families through the steps of identifying a facility and managing admission.

- Providing support as families traverse the treatment landscape.

- Communicating with treating facilities when appropriate authorization is granted.

- Coordinating the transition from inpatient treatment back to outpatient care.

- Supporting the development of a plan for continued care post-discharge.

- Identifying essential resources and supports needed after hospitalization.

Our goal is to provide continuity, guidance, and supplementary family support when outpatient therapy alone is no longer adequate.
